Posting of the Pending Application:
2. If after the (10) days, no one opposes the registration, the civil registrar shall evaluate the veracity of the documents submitted.
3. If after proper evaluation of all documents presented and investigation of the allegations contained therein, and the civil registrar is convinced that the event really occurred within the jurisdiction of the civil registry office and that said event has not been registered, he shall register the delayed report thereof.
4. The civil registrar, in all cases of delayed registration of birth, death and marriage, shall conduct an investigation whenever an opposition is filed against a registration. He shall take the testimonies of the parties concerned and of the witnesses in the form of questions and answers. After investigation, the civil registrar shall forward his findings and recommendations to the office of the Civil Registrar-General for appropriate action.
5. The Civil Registrar-General may, after review and proper evaluation, deny or authorize the registration.

• Joint Affidavit of two (2) disinterested persons
• LCRO Form 1B / 1C – Birth Not-Available / Birth Destroyed
• Negative Certification of Birth issued by the PSA
• Original copy of Baptismal Certificate, and
• Joint Affidavit of two (2) disinterested persons

• Certified True Copy of Death Certificate

• Certificate of No Record issued by Civil Registrar
• Negative Certificate issued by Philippine Statistics Authority
• A copy of the Marriage Contract obtained by the couple from the marriage ceremony with License or Affidavit of Cohabitation
• Affidavit executed by the Church/Solemnizing Officer attesting to the existence of the marriage ceremony and the reason for the delayed registration OR Affidavit executed by the couple attesting to the marriage and the reason for the late submission.
• Backside of COD (Affidavit of Delayed Registration of Death) must be accomplished and notarized or subscribed by authorized Officer
• Certificate of No Record issued by Civil Registrar
• Negative Certification issued by Philippines Statistics Authority
• Death certificate issued by the church
• Affidavit executed by the Hospital/Clinic, the nearest relative or the person having administrator or attendant at death, if it occurred in the said institution or under the care of the attendant
